Zarówno bakteryjne, jak i wirusowe schorzenia skóry występują stosunkowo często. Zazwyczaj ich wystąpienie nie wiąże się z ryzykiem groźnych powikłań, jednak ich objawy mogą znacząco obniżyć jakość życia pacjenta.
Do najczęściej spotykanych schorzeń skóry o podłożu wirusowym należą brodawki, opryszczka oraz mięczak zakaźny. Z kolei wśród chorób wywołanych przez bakterie znajduje się m.in. róża, czyrak oraz liszajec. Część z nich może wymagać interwencji dermatochirurga – pacjent może zostać skierowany np. na laserowe usuwanie zmiany.

Brodawki
Brodawki, wywoływane przez wirusa brodawczaka ludzkiego (HPV) to jedna z najczęściej spotykanych chorób skóry. Najczęściej atakują stopy, choć mogą być łatwo przeniesione na inne części ciała, w tym dłonie, okolice narządów płciowych lub twarz.
W sytuacji pojawienia się brodawek na narządach płciowych konieczne jest sprawdzenie, czy ich przyczyną jest onkogenny typ wirusa brodawczaka ludzkiego. Dlatego każda usunięta przez specjalistę zmiana tego typu musi być poddana badaniu histopatologicznemu, które pozwala sprawdzić, czy pojawiły się w niej komórki nowotworowe.
Mięczak zakaźny
Wirusowe zakażenie skóry prowadzące do powstania mięczaka zakaźnego najczęściej diagnozowane jest u dzieci w wieku szkolnym. Ma postać twardawych, gładkich, woskowibiałych lub różowych grudek. Jest to stosunkowo częste schorzenie wywoływane przez wirusa MCV (Molluscum contagiosum virus). Dzieci nie mają jeszcze wykształconej odporności na tego wirusa; dodatkowo może on się łatwo przenosić podczas aktywności związanej z bliskim kontaktem z inną osobą, np. podczas zajęć wychowania fizycznego.
W wielu przypadkach objawy mięczaka zanikają samoistnie po około trzech miesiącach, jednak wiele osób woli się ich pozbyć wcześniej. Leczenie może polegać m.in. na laserowym usuwaniu zmian (szczególnie jeśli inne metody okazały się nieskuteczne), pozwalającym na wycięcie nawet dużych zmian bez naruszania zdrowych tkanek.
Opryszczka
Opryszczka również należy do zmian bardzo często spotykanych w praktyce klinicznej. Jej przyczyną jest wirus HSV, który po zakażeniu wnika do tkanki nerwowej, gdzie w normalnych warunkach pozostaje w fazie latentnej. W sytuacji, gdy organizm jest osłabiony (np. po urazach, w sytuacjach długotrwałego stresu lub przy innych infekcjach) może spowodować wystąpienie choroby i pojawienie się zmian skórnych w postaci bolesnych pęcherzyków wypełnionych płynem.
Leczenie opryszczki najczęściej polega na podawaniu acyklowiru, jednak możliwe jest także mało inwazyjne laserowe usuwanie zmian. Co ważne, zabieg umożliwia niemal natychmiastowe uśmierzenie bólu; sama zmiana znika w ciągu kilku dni.
Czyraki
Czyrak to zmiana wywołana przez gronkowca złocistego. Jej pojawienie jest skorelowane z nieprawidłową pracą układu odpornościowego, np. w przebiegu cukrzycy, chorób nerek, AIDS lub przy leczeniu immunosupresyjnym.
Czyraki mają postać zaczerwienionych guzków, które z czasem przekształcają się w krostę i obumierają. Mogą pozostawiać blizny.
W leczeniu bakteryjnego zakażenia skóry tego typu stosuje się antybiotykoterapię. Często konieczny jest drenaż zmiany związany z jej nacięciem – tradycyjnym lub, jak ma to miejsce w gabinecie Vitabene, za pomocą lasera. Pozwala to zmniejszyć ryzyko powstania ubytku w skórze lub blizny po czyraku.
Liszajec
Liszajec to jedno z najczęstszych schorzeń wywołanych zakażeniem bakteryjnym skóry. Zazwyczaj odpowiadają za nie gronkowce oraz paciorkowce. Ma postać płytkich pęcherzy wypełnionych ropną lub surowiczą treścią. Najczęściej występuje na twarzy (szczególnie w okolicach ust i nosa), choć może przenieść się także na inne części ciała; rozsianie zmian na całym ciele spotyka się jednak rzadko, najczęściej u pacjentów z niedoborami odporności. Charakterystyczne dla liszajca są miodowożółte strupki.
Leczenie liszajca polega na podawaniu antybiotyków; nie jest to zmiana, która wymaga interwencji dermatochirurga.
Róża
Róża to zakażenie bakteryjne skóry oraz położonych pod nią tkanek, spowodowany przez paciorkowce Streptococcus pyogenes. Choroba ta atakuje naczynia krwionośne; w jej przebiegu pojawia się charakterystyczny rumień i obrzęk. Skuteczne leczenie polega na podawaniu antybiotyków.
Oprócz tzw. pierwotnych zakażeń bakteryjnych skóry istnieją także liczne nadkażenia wtórne (tzw. zliszajowacenia). Są to powikłania różnych zmian, zazwyczaj wysiękowych oraz swędzących. Leczenie chorób skóry wywołanych przez bakterie lub wirusy nie jest zazwyczaj skomplikowane. Wdrożenie terapii farmakologicznej oraz usuwanie zmian skórnych laserem daje duże szanse na skuteczne wyleczenie, również u pacjentów z grup bardziej podatnych na rozwój zakażeń niż reszta populacji.
